Hospice care includes physical, psychological, social and counseling services. The goal of hospice care is to provide care by managing pain, symptoms and promoting quality of life.

Where to Start?

Horizons Hospice will arrange to meet with you and your family to provide information on our services.

A Horizons Hospice Representative will contact your attending physician to discuss if services are appropriate for you and your loved ones.

Hospice services will begin once a plan of care is developed by you, your attenting physcian, and the Hospice Interdisciplinary Team that is specific to your needs.
