Job Title/Position: Community Education Representative

JOB DESCRIPTION SUMMARY

The objectives of this position are to assist referral sources in identifying patients with care needs that match the services provided by hospice, home health and/or DME to educate the potential patient and family regarding the services provided, to facilitate the admissions process, to communicate with appropriate referral facility staff. In addition this position is responsible to ensure that the agency and referral source is prepared for the admission through communication with agency and referral source staff.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Responsibilities of the Community Education Representative include, but are not limited to, the following:

PRE-ADMISSION ASSESSMENT AND CARE COORDINATION
  1. Provide on-site clinical assessment service to referring providers. Assessments will determine if the patient's needs appropriately match services provided by hospice, home health and/or DME.
  2. Assist referral sources with identification of appropriate patients and educate regarding admissions criteria.
  3. Be responsible to submit daily, weekly and monthly marketing reports as required in a timely manner as assigned to your position.
  4. Communicate admission, clinical and financial information to appropriate facility staff members. Ensure that the facility and the agency is prepared for the admission.
  5. Obtain the necessary information (medical and financial) to provide information relevant to the completion of the admissions process.
  6. Manage the relationship with referral sources. The position is the primary source for information to the referral sources regarding current services.
  7. Conduct regular visits to assigned hospitals and SNFs to build and maintain relationships with Discharge Planners, Case Managers, Physicians and work to ensure that the agency is responding promptly and is meeting their needs. Maintain regular visits that serve as educational opportunities for the referring facility staff.

POSITION QUALIFICATIONS
  1. Must be a licensed driver with a reliable automobile that is insured in accordance with state and/or organization requirements.
  2. Ability and willingness to travel extensively and work on-site in numerous facilities.
  3. Must have excellent interpersonal skills and the ability to relate professionally and positively to corporate staff and to work cooperatively with other employees at all levels as well as potential customers.
  4. Must have excellent documented assessment skills.
  5. Must demonstrate a work history of being a self-starter, working independently with minimum daily direction, self-motivation with goal directed sales skills.
  6. Must be capable of maintaining regular attendance.
  7. Must meet all local health regulations and pass post employment physical examination, if required.
  8. Must be capable of performing the essential job functions of this job, with or without reasonable accommodations.
WORKING CONDITIONS
  1. In and out of automobile.
